Applicant Instructions (PDF)— Supervisors, this guide will be helpful for any candidates who want to apply online through the AmeriCorps website. Feel free to share.

Sponsor Evaluation form (Word)— Supervisors please fill this form out during the interview and send it along to HFH-MN with the remaining application for processing.

What to Know Letter (Word)— Supervisors please go through this form with your selected VISTA and send along with the remaining application for processing.

In Service Year Forms

HFH VISTA In Service Training FUNDS — As part of the program budget, each VISTA member is allotted $150 of in-service training funds. These funds can be used to pay for trainings, conferences, workshops or classes that offer skill-building opportunities related to the VISTA member’s service activities.

Future Plans or Early Termination form (Word)
End of Term PPR form (Word)— For Supervisors and VISTAs, this final PPR will be due to the HFH-MN office prior to your end of service. To successfully complete the term of service, this form must be completed and submitted to the HFH-MN office.
